Form PHS-6.1 Chick Papers HINTS FOR GOOD SAMPLES AND DATA

1. Plan for samples to arrive on a week-day.
2. Use appropriate sample numbers and
containers - consult laboratory if in any doubt.
3. Package in a secure leak-proof manner.
If not possible to dispatch immediately
keep samples refrigerated (not frozen).
4. Complete this form, fold in 4 and place
in a "Documents Enclosed" pouch with the
address visible. Peel off backing and
stick to outside of package.
